What are some problems with food inspection in canada or us?

I am doing a research paper on food problems and i would like to know if there are any problems with the inspection regulations in canada or the us, that show the standards of regulation are minimal or very problematic. References would be nice (from the cfia or the fda). Because i cant seem to find anything that shows how the standards are poor but i know they are because how else are all theses food recalls happening. Also anything on government affiliation with major companies that allow the standards to be disregarded. Thank you!!!
