I Need some help with a care plan?

When I went to get Judy up this morning she had soiled herself. She needed my full istance to clean her and then shower her. On the way back from the shower she again soiled herself, and we went through the same cleaning again. She was volatile, hitting and punching me while I was trying to clean her up. When she was clean, I left her in the day room. She seems fine now, with no more problems.
